Preschool Student Appraisal

This form must be printed and given to the preschool teacher of all students applying for Kindergarten. The teacher will then return the form to the school office.

Student Appraisal

This form is required for all students applying to grades 1-8 and must be printed and given to your child's most recent school. The school will return the form to CHC.

Transcript Release

Required for all students applying to grades 1-8.

10-11 Waiver & Indemnity

Once enrolled, this form must be initialed and signed. Please turn in one form for the entire family.
